Health forms from NYU’s Health Center were recently mailed to your permanent address. These forms are:
- A self-reported Student Health History form (red scantron form). To be complete by both NYU and Visiting Students.
- An Immunization History form (orange scantron form) – requires a signature from your doctor’s office. To be completed by Visting students only.
Please return these forms to the Health Center to the address is listed on the form NOT the Office of Global Programs. The deadline for the spring is December 15, however if we receive the information by the end of December or first week of January it will be fine.
